Quest Diagnostics Foundation Announces the Quest for Healthy Communities Champion Awards – Now Accepting Nominations

Good health begins long before an individual enters a doctor's office or clinical laboratory. It begins in the spaces where we live, work, and raise our families. Across the country, dedicated community organizations work every single day to address disparities, connect individuals to preventive diagnostic care, and ensure compassionate, culturally competent support is within reach for everyone.

At Quest Diagnostics, we are driven by our purpose, "Working together to create a healthier world, one life at a time." To champion this mission and support the local organizations carrying it forward, the Quest Diagnostics Foundation is proud to announce the nomination portal is now open for Quest for Healthy Communities Champion awards.

Through this initiative, the Foundation will award $10,000 grants to selected community nonprofits that have demonstrated measurable impact improving community health outcomes.

Prior Quest for Healthy Communities Champion award honorees have helped community health centers expand weekend outreach hours, supported Community Health Worker (CHW) training programs in maternal care navigation, and provided vital infrastructure to connect high-risk individuals with preventive diagnostic testing.

What we are looking for

The Quest Diagnostics Foundation is prioritizing organizations whose initiatives directly align with 1 or more of our core impact pillars.

Advancing health: Programs actively dismantling clinical barriers to ensure every individual—regardless of zip code, background, or income—has access to high-quality care

Addressing social drivers of health (SDoH): Innovators providing creative solutions for food security, transportation, health education, and maternal-child support networks

Fostering community-clinical linkages: Organizations bringing critical care directly to those in need through mobile clinics, community health workers (CHWs), and localized diagnostic screenings

Developing the community workforce: Organizations finding creative ways to engage the communities they serve directly into the solutions they are delivering

Eligibility and selection guidelines

To ensure grant funding drives institutional continuity and community-wide scale, the 2026 Champion program focuses exclusively on organizational nominees:

Eligible entities: Must be registered US-based 501(c)(3) public charities, public health entities, or formalized community coalitions headquartered and serving in the United States

Exclusions: For-profit entities, partisan political/lobbying campaigns, fraternal/labor organizations, and programs funding individual religious proselytization are not eligible
